A Word From The Boys...

How to Use The Medical And Dental Records Binder

We are so pleased you have chosen this very unique medical and dental records binder into which you can document all items pertinent to your childs medical history.

Our very clever Mommy originally devised this method to keep track of all of our information, as no sooner did one of us recover from the chicken pox then another one got an ear infection! That, of course, meant trips to the pediatrician and to the pharmacy. With three boys all in diapers, it became clear to her that all of this information could be not kept on a shopping list!

Our Mommy recommends you take the binder with you whenever you take your child to the doctor, hospital, or emergency room. That way, you will have all past and present information at your fingertips. The last thing you need to rely on when you are in an important situation is your mental notes. Just jot things down in this handy binder in the pertinent sections. Also, if you plan to go out of town and have someone else watch your child, be sure they have access to this binder. If an emergency ever arises, they will have all the information they need with respect to your childs medical history, plus the insurance information, and a medical release form! You can keep your mind at ease if you keep this binder updated. (Well, at least that part of your mind at ease...we kids tend to make most parents minds a bit crazy! Just ask our Mommy!)

Please note the following sections have a few of the same pages: Illnesses, Medicines, Radiology, Allergies, Dental, and Special Medical Conditions. In order to ensure you have ample pages to fill out over the years, please photocopy several of each pace before you fill them out, or, you can contact our Mommy and order more to match your book Whenever we get sick, even if it is with the common cold, Mommy always makes a notation of it in our book...how long it lasted...what medicines we took, etc. Over time, it has become a handy reference for her. That is now why we call her "Dr. Mommy!" It is also very important to keep track of any and all medications you give to your child. Even when Mommy just gives us Tylenol, she makes a note of it. You just never know when such information might be necessary.

Also, before you put in the spine sheet located in the inside binder pocket, be sure to write or type your childs name on it. In order to easily put the spine sheet in the vinyl pocket on the outside of the binder, lay the binder flat, face down, and then slip the sheet in!

Another important feature in this binder is the identity page. be sure to keep an updated photo and fingerprints of your child there. Experts indicate that every 6 months to one year, the photo and fingerprints should be updated. Around age 13, fingerprints have generally matured, and it should not be necessary to do that again after that point. Our Mommy has no trouble getting our fingerprints... .they can be found on anything from windows to mirrors.

Well, we hope you like this binder. Most of it is self-explanatory. If you have any questions or comments, please contact our Mommy. She would love to hear from you! Be sure that once your children are all grown-up and leave home, after you kiss them goodbye and slip some cash (Cmon....you have to do that!), you give them this binder so they can have access to their complete medical history! It will be one of the most important gifts you can ever give them.

Thank you for making this investment in your child.

Healthfully yours,

Alec, Zachary, and Noah Butenas
