You know the drill. The nurse enters and inevitably proceeds to question you on every ounce of medical history your child has ever experienced, and you're expected to rattle it off as if you have a doctorate in pediatric medicine. Well, thanks to a new medical and dental records manual designed to document every piece of your childs medical history, "20 questions" at the doctors office can now be a manageable task. Ann Butenas, owner of ANZ Publications, created the unique medical document keeper and said that raising three young boys of her own has been the inspiration behind the book. "Attempting to keep track of all that information is just too hard," she said, "and its almost impossible with multiple kids." The manual is divided into several categories, with pre-printed forms, that makes it very easy to use, including immunization information, illnesses, allergies, medical release forms and insurance information, just to name a few. According to Butenas, the manual covers everything from A to Z.
