Medical Record Book

Ann E. Butenas of Overland Park thinks everyone has something for the record book - the medical record book, that is. Butenas, the daughter of a physician, knows the importance of keeping up-to-date medical and dental information, and she has designed a notebook to help parents keep track of their kids medical histories.

Its called, simply, "The Medical & Dental Records of (fill in the childs name here)." Its a loose-leaf binder, organized by category (immunizations, well-child exams, allergies and so on), with easy-to-fill-out pages that guide you through the information you should have. Some pages have pockets for holding extra papers.

With this binder, you're better prepared for a childs medical emergency - and for the string of health forms that kids need for school, camp and sports. The medical and dental records notebook costs $24.95 plus shipping and handling ($5.50 for the first book, $3.50 for each additional book) from ANZ Publications.
